VSNA Scholarship Form 2008 Application Deadline May 31, 2008 I ________________, Veerashaiva High School Senior college student am residing in North America. I am applying for the Scholarship program to pursue college education. I enclose the following documents, which I understand must be submitted for eligibility.  1. Evidence of VSNA Membership  My parents are VSNA members since (year) ___________. My parents __________belong to ____________chapter since _______________ year.  2. Evidence of Veerashaiva Community Service Letters of Recommendation from the chapter president regarding the following activities: Involvement in community activities, conferences and attend chapter activities. Evidence of similar activities in general also be submitted, however, preference will be given to VSNA community service. 3. Qualifications Academics - Demonstrate academic initiative Service - Provides dependable and organized assistance selflessly Works well with others Volunteers in community activities responsibly 4. Two supporting letters from school From School A Teacher 5. Name of the college you plan to attend 6. An essay explaining why you should be selected for this scholarship (one page ) 7. Parent Endorsement: I am a Central VSNA member for the current year.
