VSNA Awards Form 2008 Application Deadline May 31, 2008 I wish to nominate the following person for one of the following award. Two recommendation letters from two VSNA members (Other than submitter and family members) The affirmative vote of two thirds of the Executive Committee shall be required for approval No more than one award will be presented to the selected individual at the annual conference by President of VSNA. If the nominees do not meet the criteria, no such awards shall be given for that year. Criteria for VSNA Awards: Outstanding Service Award · The nominee should be the Life member or a member in good standing for at least 3 consecutive years · The nominee should have a keen interest in the activities of VSNA and have served in the chapter or central office. · The nominee should have committed to the primary goal of VSNA and have rendered selfless service to the VSNA · The nominee should not have held office in VSNA in past 3 years. Professional Achievement Award · The nominee need not be a Life member or a member in good standing. However, the nominee should be a Veerashaiva. · The nominee should have been recognized in his/her professional field at state/national/international level · The nominee should have a keen interest in the activities of Veerashaiva and contributed to the success of our community. · The nominee should have committed to the primary goal of Veerashaiva and have rendered selfless service to the Veerashaiva community.
